Assessment
This is an extended appointment with Dr. Worley that includes a detailed review of your history, an assessment of expanded biomarker testing results, a detailed visit note with lifestyle and supplement recommendations, and a 1 to 2 week follow-up appointment with our health coach.
This is a one-time assessment that is required before joining a membership plan.

Plans
Our membership plans are for those who have completed the Foundational Assessment and who want to take a deeper look into the upstream cause(s) of their symptoms. It is designed to provide the support and education needed to achieve lasting and durable improvements in your health. During the Foundational Assessment, Dr. Worley will discuss whether he feels your goals align with our practice model, if you may benefit from ongoing care, then recommend either a 12 month or 6 month plan. Note that completion of the Foundational Assessment does not guarantee participation in a membership plan.
